Investigation Summary
On February 10, 2009, Employee #1 was welding a water storage tank. He stepped off a hook ladder onto the scaffold walking and working surface. The 2-in. by 10-in. pine plank broke, and he fell approximately 125 ft to the bottom of the tank. He was killed.
