FATAL TRUCK/FLATBED TRAILER RAN OVER EMPLOYEE

On November 5, 2008 at approximately 10:40 am an accident occurred at the Wastequip facility in Durant, OK. Employee #1, a truck driver, had returned to the yard due to brake problems with the trailer. Employee #1 contacted the supervisor at the shop stating he was returning to the yard to have his load transferred to another trailer. After arriving at the yard Employee #1 met with the supervisor, the supervisor then went out to get the forklift to move the trailer to which the load would be transferred to. The supervisor saw Employee #1 walk in the direction of his truck. Not knowing that Employee #1 had crawled under the trailer to check the brakes. The supervisor attached the forklift to the trailer and started moving it forward. At that time Employee #2 who was also operating a forklift in the area, tried to get the attention of the supervisor when he saw Employee #1 trying to crawl out from under the trailer. Employee #1 had been run over by the time the supervisor stopped moving the trailer. Employee #1 died.
