Employee Is Killed When Run over by Dump Truck

At 7:30 a.m. on July 22, 2008, Employee #1, a bulldozer operator, came to talk to a dump truck driver on the driver side while the truck was backing up onto the job site. Employee #1 was there to tell the truck driver where to dump the rock. Employee #1 then walked to the front of the truck, around the other side, and to the back of the truck. Employee #1 was acting as the truck driver's spotter at that time. Before backing up the truck, the truck driver looked at the left side mirror, then the right side mirror and saw Employee #1 on the right side about 10 feet from the truck. The truck driver kept backing up and when he looked at the left mirror again he saw a white helmet flying. He threw on the air brakes and parking brakes. When he got out of the truck he saw Employee #1 on the ground with his head crushed between the tires, fatally injured.
