Investigation Summary
On February 27, 2010, Employee #1 was standing on the sixth step of an 8-ft stepladder with both feet and was preparing to clean a cooling fan. Employee #1 lifted his left foot and was trying to place his left foot on the top cap of the ladder. The ladder tilted and Employee #1 fell of from the ladder to grade. Employee #1 sustained a head injury and back pain. Employee #1 was hospitalized.
