Investigation Summary

Investigation Nr: 201509189
Event: 04/01/2009
Employee Is Injured When Pinned Between Moving Parts

At approximately 8:15 a.m. on April 1, 2009, Employee #1, a concrete pump operator/oiler, was oiling the inside drive train of the FMC Link Belt crawler crane, Model Number B-310. During the oiling of the crane, Employee #1 was instructed by the site Foreman to perform another task. After completing the secondary task, Employee #1 returned to finish oiling the crane; however, he did not notify the crane operator upon his return. Unaware that Employee #1 was working on the crane, the crane operator maneuvered the cab of the crane counterclockwise, pinning Employee #1's upper body between the undercarriage of the catwalk and its right side track. Employee #1 was taken to Highland Hospital, where he was hospitalized and treated for lacerations to his face and back of his head, requiring several stitches and staples, as well as multiple fractured ribs.
