Investigation Summary
At approximately 4:00 p.m. on October 6, 2008, an employee, a mechanic, was replacing a hydraulic hose for the reach mechanism on a customer's Crown Electric Forklift, (Model Number 35RRTT and Serial Number W-85374). He raised the forklift mast up, but during that process, the hose was stuck. The mast of the forklift was not blocked or chained, and the forklift was not deenergized. He proceeded to reach his hand into the mast to untangle the hose while standing on the operator's platform, when his stomach pushed the control lever activating the mast to lower onto his arm and crushing it between the carriage and frame. The employee sustained a deep right arm muscle laceration which required surgery and hospitalization for two days at Eden Medical Center.
