Investigation Summary
On August 25, 2008, an employee was installing cedar wood siding on a two story dwelling. The employee and coworkers were working on ladderjack scaffold that was damaged and not secured from displacement. The ladder slipped, and the employee fell approximately 13 feet to the ground. The employee was not protected from fall hazards by personal protective equipment. The employee hospitalized and treated for fractured ankle.
