Employee Amputates Finger in Moving Blade

On February 8, 2010, Employee #1 as adjusting the sensors on the Chamfer #2 machine (G.E. 2002 Chamfer). A chamfer cuts grooves in wood. The chamfer was turned off and locked out but the blade continued to coast. Employee #1 reached in to make an adjustment to the sensor and the still spinning blade cut his finger. Employee #1 suffered a partial amputation to his right pinky finger. The employer was Commercial Lumber & Pallet Co. Inc., a pallet manufacturer. The cause of the accident was a failure to stop and deenergize or disengage the machine, and the moveable parts had not been mechanically blocked or locked out. The employer reported the accident to the West Covina District Office at approximately 12:25 p.m. on February 8.
