Investigation Summary
At approximately 10:30 a.m. on July 29, 2008, Employee #1, an independent truck driver, parked his truck in a location he was not supposed to for unloading. An operator of a high lift fork truck was driving backwards with the spreader bar, approximately 20 ft wide at the low position instead of at the high position. Employee #1 exited his cab to warn the forklift operator and was struck by the spreader bar. He was hospitalized with fractures to his ribs and legs.
