Employee's Four Toes Are Amputated When Foot Is Smashed

On January 3, 2008, Employee #1, a trainee, was left alone on fork truck. Since he could not stop it, he put his left foot out to help bring to a standstill. Employee #1 smashed his foot. He was hospitalized, and four toes on his left foot were amputated.
