Investigation Summary
On May 2, 2008, Employee #1, of Aikins Distribution, Incorporated, performing his normal work duties, was on a loading platform. For some reason, he fell and struck his head. The loading platform was 8 ft wide and 54 in. high. Employee #1 was taken to the hospital for treatment of a severe concussion, where he later died.
