Employee Is Burned When Exposed to Electric Shock

On November 3, 2008, Employee #1 and a coworker were running a new cable into a 277/480-volt electrical box that was energized. The bare copper wire came in contact with the copper bus and grounded to the panel, causing an arc flash. At that time, Employee #1 was standing on a ladder in front of the panel and sustained first, second, and third- degree burns to his face and arms, and also to his upper torso. The panel was not locked out as it was required. In addition, the employees were not using any means of insulation or grounding of the circuits.
