Investigation Summary
On June 17, 2008, Employee's #1, #2, and #3 were working on a work platform elevated approximately 9 ft by an all terrain forklift. The work platform was not secured to the lifting carriage and fell. Employee #1 suffered a laceration and was hospitalized. Employees #2 and #3 suffered minor injuries to their hands.
