Employee Is Killed in Fall from Unguarded Wall Opening

On January 19, 2009, Employee #1, of Midwest Insulators, was inspecting an elevated work area associated with a new construction. He fell through an unguarded wall opening to a doorway leading to a stairwell. He was killed from head injuries he sustained in the fall.
