Investigation Summary
Employee #1 was assisting another tractor trailer operator attaching a hose to the trailer. Employee #1 placed himself under the trailer just prior to the operator moving the tractor trailer to reposition the trailer so the hose would reach. Employee #1 was run over by vehicle and was killed.
