On October 23, 2008, Employee #1, a machine operator with Penn Engineering & Manufacturing Corp., was making screws. While loading a Davenport Automatic Screw Machine (WD-130, Serial Number 13301, Model B), the tip of his left index finger was amputated. Employee #1 was hospitalized for treatment.
