Employee's Finger Is Amputated by Conveyor

On March 17, 2008, Employee #1, a maintenance man working for a large meat packing plant, was notified that a conveyor belt needed repair. Employee #1 and his direct supervisor, who worked for the same company, went to the area. Employee #1 asked his supervisor if he needed to lock and tag out the conveyor. The supervisor stated that he would watch the switch that turns on the conveyor and not to worry. Apparently, the "OFF/ON" switch was located approximately 40 feet away, up 18 stairs, and 25 feet from the other side of a closed door. Apparently, other doors also provided access to the "OFF/ON" switch. Another supervisor entered one of the other doors and asked the employees in the area if they knew why the conveyor belt was off. None of the employees knew. The supervisor turned the conveyor belt on, while Employee #1 had his right index finger in the mechanism, and his finger was amputated. Employee #1 was taken to the hospital, and he was hospitalized. Both Employee #1 and his supervisor were terminated because they had not followed lockout/tagout procedures.
