Employee Is Killed When Ejected Form Aerial Platform

At approximately 10:00 a.m. on September 10, 2008, Employee #1 and a coworker were changing a traffic signal light bulb at an intersection. Employee #1 was using a traffic maintenance truck, equipped with an articulating tower, along with a traversing platform, to reach the traffic signal. He was raising the platform into an open lane of traffic and was not wearing a safety harness. A semi-truck and trailer struck the platform, and Employee #1 was thrown from the platform, approximately 35 ft. Employee #1 was died.
