Employee Losses Balance, Falls and Dies Later

On September 8, 2008, Employee #1 was unloading a skid of ladders that were 10.4 ft wide, which is too wide to fit into the open space on the mezzanine on the east end. He reached for a falling box from the skid that he was attempting to unload on the mezzanine. The skid was in a cocked position preventing the gate from closing. In reaching for the falling box, he lost his balance, and fell head first over the open edge of the elevated storage area. The edge of the skid was sticking out over the edge of the storage area. He fell 8.83 ft and died ten days later in the hospital.
