Investigation Summary

On July 10, 2008, Employee #1 was send by his supervisor to mow the grass on the soccer field and to move two piles of dirt by the soccer field. Employee #1 was using the John Deere Tractor, Number 4300, which the school owned, to push a large tree limb out of the way of the pathway, used by the cross country team, when running the track. The limb came down during a storm the night before. It appeared that Employee #1 tried to lift the limb and push it at the same time, when the limb came back onto the tractor, over the bucket that was mounted on the front of the tractor. The limb struck Employee #1 in his chest and then came to rest on his neck, pushing his head back. When he was found, it took two people to pull the limb from Employee #1. One of them was trained in cardiopulmonary resuscitation and tried to administer it to Employee #1 till the emergency medical services personal arrived. The emergency medical services and the supervisor both arrived at approximately the same time, at about 1:00 p.m. The emergency medical services personnel tried to revive Employee #1, but they were unable to do so. The coroner of Whitley County stated in his report that it was an accident.
