Event: 07/23/2008

On July 23, 2008, Employee #1 was centering coil on roll line for an automatic coil stacker to lift up coil. A sensor detected the coil and the crane grab leg came down to lift up the coil. The employee was bent over the safety rail. He was caught between the rail and the grab leg of the crane. Employee #1 was crushed and killed. There were danger pinch point signs posted in multiple locations on the Auto-crane. Employer stated company rules and training does not allow any employee to be in pinch point area during automatic crane lifting operations; however, there was no machine guarding preventing employee from entering high hazard pinch point area.
