Investigation Summary
On March 24, 2008, Employee #1 was writing the weight of a bale of cardboard on the front of the bale. There were three bales stacked and Employee #1 was writing on the top bale while apparently standing at ground level. Employee #1 was working alone when he apparently fell and struck his head. He died.
