Employee Is Injured in Fall from Scaffold

On April 22, 2008, Employee #1, a carpenter foreman, was walking on undecked joists, which were part of a large area suspended access (scaffold) system known as a "QuikDeck." He was also outside of the guardrail system and did not have the lanyard of his personal fall arrest system attached to an anchorage. Employee #1 fell from the joists approximately 29 ft to a concrete floor and was hospitalized with serious injuries.
