Employee Is Killed When Struck by Pipe

At approximately 1:15 p.m. on April 15, 2008, Employee # 1 and a coworker, the forklift operator, were unloading pieces of pipe from the bed of a 40 foot truck. Each piece measured 18 inches in diameter, 42 feet long and weighed approximately 2,800 pounds. Employee #1 was standing at the edge of the truck bed rolling the pipe pieces onto the forklift. Employee # 1 lost his balance causing him to fall backward onto the ground. A pipe piece subsequently rolled backward and off the truck bed striking the body of Employee # 1. Employee #1 was killed.
