Event: 09/25/2008
Employee Sustains Head Injuries, Later Dies

On or near September 25, 2008, Employee #1, a helper at Golf Harvest Seafood, sustained head injuries. No one witnessed how the injuries occurred, and the investigators could not determine how they occurred through observations or interviews. Based on interviews, a maintenance employee found Employee #1, lying on the floor, at the entrance of the basket room on September 25, 2008. Employee #1 was breathing hard, shaking, foaming at the mouth, and moving his eyes from one side to the other. The maintenance employee attempted to aid Employee #1 by rolling him over on his side. Employee #1 returned to the original position of lying on his back. The maintenance employee finally placed an object behind Employee #1's back to prevent him from rolling back to his original position. At approximately 4:35 p.m., the maintenance employee called emergency medical services. Paramedics arrived and found Employee #1 convulsing and experiencing seizure activity. Employee #1 was transported to a nearby hospital and was later transferred to a regional hospital via Life Flight. He died from his injuries two days later.
