Investigation Summary
On March 30, 2008, Employee #1 was working in a packing facility, where he was instructed to wash down an out-going conveyer. Employee #1 attempted to locate the cut-off switch for the conveyor; however, was unfamiliar with the system and was not successful in doing so. Employee #1 decided to wash the conveyor while it was still running. While holding the hose with his right hand, Employee #1's left hand contacted the conveyor. The glove on this hand became caught in the nip points of the conveyer, pulling his hand and arm into the conveyer. Employee #1 struggled to pull his arm free and when finally separated from the machine was amputated 4 inches below his shoulder. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injury.
