Employee Is Injured in Fall from Powered Industrial Truck

On February 18, 2008, Employee #1 was riding the forks of a power industrial truck in a warehouse to reach a storage area that was approximately 18 feet above the concrete floor. As Employee #1 was elevated to about 16 feet, he slipped and fell from the truck. He was not wearing any personal protective equipment. Emergency services were contacted and, upon arrival, Employee #1 was taken to CMC Main, where he was hospitalized and treated for a laceration to his head and fractures to his vertebrae.
