Investigation Summary
On February 13, 2009, an employee was working at an alternate location at the waste water treatment facility jointly operated by the ER and the City of Corning. The employee was disconnecting a 24V battery from the charger. It exploded, causing burn and laceration injuries to his eyes, face, and neck. He was not wearing PPE, at the time of the accident, but it was available. A supervisor, Team Leader, was the only other person on-site, at the time of the accident. According to the supervisor, he had performed a new hire orientation with the employee when he started at that location in 2007, but the orientation did not include the battery charger. The battery charger is used infrequently, just a few times a year, so they did not provide any specific instruction on the equipment and did not have an specific PPE requirements while using it.
