Employee Is Killed When Platform Collapses

On March 18, 2009, Employee #1 and two coworkers were attempting to set a 6-in. by 12-in. by 16 ft long beam into an overhead bracket. They were standing on planks supported at each end by 2-in. by 6-in. boards nailed perpendicular to the planks between vertical posts. The planks were 14 to 16 inches wide, approximately 14 ft above the ground. The sides and edges of the planks were not protected. Employee #1 and the coworker were not wearing any form of personal fall protection. As the beam was being maneuvered into place, the outside 2-in. by 6-in. board supporting the working platforms broke, and Employee #1 and a coworker fell 14 ft. The coworker was not injured. Employee #1 struck his head and died.
