Employee's Finger Is Amputated in Press

On March 18, 2009, Employee #1, a production worker, walked over to Flare Press Number 4 to get production numbers from the press operator. The in-house made hydraulic press accepts circular metal stock with one end held in place via two clamping guides that move together until they encase the bottom of the stock. The top of the stock is flared via a die on the upper ram. Employee #1 saw a quality issue on the piece of stock the operator had just placed into the press. He reached over the operator to point out the issue, pointing with his gloved right index finger. The operator activated the press, and the two clamping guides came together clamping Employee's finger between them. Employee #1's finger was amputated.
