Empolyee Falls from Ladder and Injures Head

At approximately 1:15 a.m. on October 11, 2008, Employee #1 and another employee were servicing a local fast food restaurant at the accident site. Employee #2 was finishing cleaning the hood, on the roof, handing equipment down to Employee #1, who was standing on the 2nd or 3rd rung of the ladder. Employee #1 stated he was trying to tell Employee #2 he did not have a hold of the 2-3 gallon spray bottle yet when Employee #2 let go of it. Employee #1 inadvertently let go of the ladder to shield himself from the falling bottle, causing him to fall to the concrete below and hit his head. Employee #1 stated he was never on the roof and never higher on the ladder than the 2nd or 3rd rung. He had maintained 3-points of contact while handling equipment, until he let go to shield himself. The ladder was a fixed ladder and was found upon inspection by the Division to be in compliance. The employer was an industrial cleaning service.
