Investigation Summary
Employee #1 was working alone at the time of his accident. Employee #1 reported to his assignment and took a ladder off his service truck for use in climbing a telephone pole to complete his work. Employee #1 leaned the ladder against the pole but did not secure it to the pole. Employee #1 admitted he was trained to use a ladder strap under these conditions but the employer never issued the ladder straps after the training. Employee #1 stated he hooked his climbing belt around the pole and began his climb. When he reached the height of the strand, Employee #1 unbuckled the belt to allow his ascent above the strand. Employee #1 then tried to rebuckle the belt but his glove was in the way. At the same time, Employee #1 attempted to make the transfer from the ladder to the pole but the ladder shifted and his left gaff pulled out of the pole. Employee #1 fell approximately 16 ft to 18 ft to the ground. Employee #1 sustained an unspecified fracture and was hospitalized.
