At approximately 11:30 a.m. on September 10, 2008, Employee #1 was fueling his truck at the end of the shift. He collapsed and hit his head on the concrete, fracturing his skull. Employee #1 had a previous head injury that may have caused the sudden collapse. Employee #1 was hospitalized.
