Investigation Summary
On March 10, 2008, Employee #1, a laborer, was working on the erection of a systems-engineered metal building. The erection of the building had been completed with the covering of the roof beginning the day of the accident. Employee #1 got onto a small portion of the roof covered with metal panels and walked down towards the eave (18-ft fall distance) to reach a roll of insulation encased in plastic. Employee #1 grabbed the roll and began dragging it up the roof towards the ridgeline (24-ft). About half way up the roof, one end of the roll snagged on a roof fastener causing Employee #1 to lose his balance and fall onto the roof. As Employee #1 fell to the roof, he began to slide, and fell off the leading edge approximately 21-ft to the interior slab. Employee #1 sustained serious trauma to his body including multiple fractures and was hospitalized for 12 days.
