Investigation Summary
On December 16, 2008, Employee #1, a mechanical power press operator working for Unique Metal Design, Inc., was using a turn-over bar to make an adjustment to the dies of a full-revolution mechanical power press, when his right hand was crushed by the dies. He was hospitalized with his injury. Further investigation revealed that the press was equipped with two-hand trips and a foot pedal but the two-hand trips were not functioning so the press was being operated with the foot pedal. In addition, the machine was not guarded and was not deenergized while the employee was adjusting the dies. The employee was also not properly trained in the operation of the press.
