Employee Suffers Blunt Head Trauma in Fall from Ladder

At approximately 12:35 p.m. on August 21, 2008, Employee #1 was using a ladder to detect an odor in an overhead compartment. Witnesses saw the employee ascend on the ladder to the cap. The ladder fell from beneath him, he lost his balance and he fell, approximately 7 ft, striking his head on a handrail. Employee #1 suffered blunt trauma injuries to his head and was hospitalized. Contractors working in the facility were using the ladder to install fire protection equipment had left the ladder at the facility. There were warning labels on the ladder cautioning users of the hazard. The employee was never instructed to not use the ladder or to use the ladder safely.
