Employee Is Killed in Fall from Elevated Platform

On May 19, 2008, Employee #1 and a coworker were working from a platform supported by a rough terrain forklift. They were not utilizing fall protection equipment. When the tipping capacity of the forklift was exceeded, the rear wheels momentarily raised off the ground, the basket tipped forward, and then returned to its upright position. Employee #1 was thrown out of the basket and landed on a concrete surface. He was killed.
