Employee Is Killed When Struck by Falling Object

On September 15, 2009, Employee #1, a painter, painted open web steel joists by dipping them in a dip tank. Each joist was 6.5 ft tall by 8-in. wide by 53 ft long. The joists come to him on a roller line. He would use an over head crane to lift two joists from the roller line and place them side by side. He then would strap them together, and then dip them in the tank. He then placed them on a flat bed trailer. He would continue this until five or six sets are placed on the trailer. These pairs of joist were not secured on the trailer until all five or six sets are loaded. While securing the bundles to the trailer, one of the banded pair of joists slid off the trailer, bottom first, and struck him in the chest and abdomen. Employee #1 was killed.
