Investigation Summary

Investigation Nr: 200411247
Event: 07/10/2010
Three Employees Receive Severe Burns in an Explosion

On July 10, 2010, Employee #1, #2 and #3 were removing a jammed roller on a salt belt recovery system. The movement of the roller caused friction between the parts of the system and the deposited chlorate crystals and finely divided titanium metal in and around the shaft end, which ignited the reactive mixture and produced an explosion and fire. All three employees were seriously burned. Emergency medical personnel were summoned. They transported the three employees to the burn center of a medical center where they received treatment and were hospitalized.
