Investigation Summary
On March 14, 2008, Employee #1 went into a guarded area to mark a piece of steel pipe that was 24 inches in diameter by 42 feet long. The pipe had not been beveled by the beveling machine. As Employee #1 was marking the pipe, a second pipe, which weighed 5,000 pounds, rolled down and pinned him between the two pipes. Employee #1 was killed.
