Investigation Summary
On February 20, 2008, Employee #1 and a coworker were hooking up an attachment to a skid-steer loader. The top two points had been connected but the bottom two were not. Employee #1 operated the hydraulic controls while standing on the loader's front tire, outside of the cab. When the attachment moved, it crushed Employee #1's chest and killed him.
