Investigation Summary
On March 3, 2008 Employee #1 of Express Personnel Services was installing a suspended ceiling at a height of approximately 19 ft. above the concrete slab floor of the cold storage unit. Employee #1 sustained a series of injuries as a result of the 19 ft. fall when the rigid insulation he was standing on failed to support his weight. Employee #1 was taken to the nearby hospital and later transferred to Harborview, where he passed away.
