Employee suffers burns to his face and hands

On January 16, 2008, Employee #1 was installing a metal cover to a meter panel was using a pointed hex head sheet metal screw to attach the cover to the front of the panel. The point of the screw penetrated the outer insulation of an energized 240 Volt conductor resulting in an arc/flash. Employee #1 was not wearing PPE and received 1st and 2nd degree burns to his face and hands.
