Investigation Summary
On July 30, 2008, Employee #1 was removing a gate from the asphalt rock bins. Employee #1 noticed an 18-in. piece of steel. The steel dislodged and pinned Employee #1's right little finger. Doctors attempted to save the finger but on August 6, 2008, the top of Employee #1's right little finger was amputated. Employee #1 was not hospitalized.
