Investigation Summary
On March 14, 2008, Employee #1 was using an electric pallet jack to move material in the work area. While moving the pallet jack Employee #1 struck his thumb on a "fixture", catching it between the handle of the pallet jack and the fixture resulting in an amputation of the thumb.
