Employee Is Killed in Fall from Bucket Truck

Employee #1 fell from the passenger side bucket while exiting the bucket. There were no witnesses to the fall. The fall potentially ranged from approximately 6-ft to approximately 12-ft 6-in. Employee #1 finished guarding a single phase primary wire for a tree trimming operation. Witnesses stated that Employee #1 was wearing his harness and lanyard until the bucket was cradled. Coworkers were not observing Employee #1 while he was exiting the bucket. Employee #1's coworkers heard a thud on the pavement and observed Employee #1 lying on the pavement. Employee #1 was killed.
