Investigation Summary
On March 16, 2009, Employee # 1 was drilling anchor bolt holes for a new metal clip welded to a column. Another company was installing metal bar joist adjacent to the column Employee #1 was working on. A rough terrain forklift equipped with a jib was used to lift the bar joist. While the fourth bar joist was being erected, the 1-in. wide, 8 ft long synthetic web sling was cut by the top flange of the bar joist. The 530 pound bar joist fell and struck Employee #1 on his head. Employee #1 was killed.
