Investigation Summary
On November 9, 2006, Employee #1 entered between the rollers of a Package Maker Machine and to straighten the stock without locking out and tagging out the machine. When the ram actuated, he was caught between it and a stack of lumber. Employee #1 was hospitalized for 10 days for treatment of fractures. He died from complications of his injuries on October 6, 2007.
