Investigation Summary
At approximately 2:00 p.m. on June 19, 2008, an employee, a Millman, was operating a Gannomat dower inserting machine, when he observed some material or objects on the machine and reach in with left hand to clean off. The employee's finger was punctured by the 8 mm dowel into the left index finger (tip). The power to the machine was not turn off. The employee's finger was fractured but he was not hospitalized.
