Investigation Summary

Investigation Nr: 202527669
Event: 03/29/2008
Employee Fractures Head and Suffers Concussion in Fall

On March 29, 2008, Employee #1 was working in a hardware store. While relocating a three-step tilt/roll ladder from one isle to another, he fell backwards and hit his head on concrete floor. Emergency services were contacted and, upon arrival, Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for a cracked skull and concussion. During the accident investigation it was identified that Employee #1 was not on the ladder during his fall. Instead, he was on the ground; however, witnesses reported that Employee #1 appeared tired and red in the face before the accident occurred. One witness mentioned that they thought Employee #1 fainted before falling backwards.
